Service Description

End of Life Pre-Planning can happen at any stage of life, but it is encouraged that it's done sooner than later. This process allows you to take agency in what happens at your end of life journey. We will go over and fill out an Advance Directive, appoint who your Healthcare Proxy and/or POA (power of attorney), discuss what a Living Will is and how to fill one out, fill out a DNR and go over why you would or wouldn't want one and go over what other services are beneficial and accessible to you and your family at the end of life.
